Toronto's Lourdes Gurriel Jr. hit what ESPN reported to be a 105.3 mph shot at Alvarez, who bravely recovered to get the out at first base before he collapsed to the ground in pain. A cart assisted him off the field.
Toronto defeated Philadelphia 3-2 in Buffalo.
Alvarez, 31, has been Philadelphia's best reliever during the pandemic-shortened campaign. Following Thursday's out, he holds a 1.42 ERA and six strikeouts in eight appearances.
